Here are my additional comments on the text:

p. 25, first sentence under ResourceName: It should read "... a sequence of
strings ..."

p. 25, last but one line on the page: should read "... cannot be modified or
controlled by ..."

p. 30, paragraph describing replace_evaluators(). Insert the following sentence
(it is almost equivalent to the one from the description of add_evaluators())
before the last one in the paragraph: "The addition of evaluators to a
ResourceName which previously had none results in the provided list of
evaluators being the only evaluators consulted on an access decision." 
Otherwise, the behavior of replace_evaluators() is underspecified in case when
no evaluators are assigned for the resource.

p. 30, paragraph describing set_default_combinator(), second line: replace word
"applied" with "specified" to preserve consistency with the description of
apply_combinator() and to avoid possible confusion.

p. 33, last paragraph: replace DecisionResults with DecisionResultList
