Ms-word version of the spec with my changes, appendix6 + IDL

I have all my action items done and incorporated in the submission text
attached. I kept revision marks so the editor (Carol) can see the
changes to the submission text.
Carol, once the IDL is completely agreed upon, you want to update the
appendix with complete IDL. I'm attaching the IDL file that has all
exception-related changes (+ some minor editorial corrections). So
please use this version for further changes. All other IDL changes
across the sections which are related to exceptions are up to date.

I also inserted the pictures, John wrote about, into his revision of use
case appendix and I swapped that appendix in the main text so that Carol
does not have to do it. Just in case, I'm attaching the appendix with my
pictures too.

I checked the IDL on visibroker for java (although "it does not mean
anything", if I can quote Inprise Jeff here :).
