HRAC outstanding and resolved issues list

Hi,

Attached are the lists of outstanding and resolved issues. I updated them
according to the feedback received at the HRAC presentation in Burlingame.
The lists should contain all issues that, as we know, exist with the current
version of the submission.

Please let me know if there are any missing or mis-represented. I'll be glad to
fix it.
