[Initial Submission] 9/30 draft

Hi,

Attached is the latest draft.  It includes a number of editorial changes (I
fixed Juggy's copyright before he discovered it wasn't there  :-) and I
fixed some missing parameters in a couple of methods related to Dynamic
rights and I tried to clarify the way Rights are used in the definition of
the structures with some tables.  Who knows what other errors I introduced.

I'd like to thank John Barkley for his questions(sent privately).   It is
wonderful to get a fresh view and his questions made me realized where the
model in my head was (is) not clearly explained in the text... hopefully
this helps but I didn't get all the explainatory text I sent him into the
draft.
